- REGISTRATION
-
School Age Center (SAC) and the Child Development Center (CDC) are the central point of contact for registration and participation in all CYS Services currently.
The SAC and CDC staff are here to assist you in placing your child in the location and facility that best meets the needs of you and your child and/or register your youth for a sports season/camp.
To register your child for any of our childcare options, you must complete and submit a registration packet. You can get registration information via email by calling COMM: 0503-363-2265 / 2266 / 2262. You can also come into the SAC, Bldg. 252 or CDC, Bldg. 223 located on Camp Walker.
A Youth Sports Physical must be completed annually, prior to the start of the sport season.
The form must be completed with a doctor’s signature and stamp and submitted to a CYS Admin during registration.
Suggestion: The best time to get your child’s physical, if possible, is in July so your child’s physical will be valid for all sports for one full year.
